- Turn on TwoFactorAuthentication system pref
- Go to patron account with staff permissions > Manage two-factor authentication
- Click enable.
- Scan QR code with authenticator app. Type in PIN code from app into Koha.
- Log out. On login page, enter username and password.
The page with the screenshot finally appears.  

Assumed this was impossible because of the human action with authenticator app; please correct!
